Job Summary Hathorne Hill Rehabilitation and Healthcare Center, a leading provider of skilled nursing and rehabilitation services, is seeking a compassionate and experienced Social Worker to join our dynamic team. As a Social Worker LTC, you will play a vital role in providing personalized care and support to our residents, their families, and the community we serve. Responsibilities Provide social and emotional support to residents, their families, and significant others Conduct assessments to determine resident status and needs, and develop personalized care plans Implement and evaluate social services programs to ensure residents receive the highest level of care and support Coordinate family council activities and maintain relationships with private and public social, health, and welfare resources Participate in resident care conferences and develop close working relationships with the healthcare team Identify and document changes in resident behavior, affect, or personality, and advise the appropriate personnel Prepare residents and families for discharges, and assist in resolving resident conflict situations Requirements Associate's degree in social work or a related human service field (minimum required); preferred bachelor's degree in social work from an accredited college/university At least one year of social work experience in a healthcare facility Strong written, verbal, analytical, and people skills Ability to read, write, and comprehend English; ability to follow oral and written instructions Ability to work hours as scheduled, with flexibility to adapt to changing circumstances Comfortable working in a fast-paced environment with a focus on customer service Ability to lift a minimum of 25 lbs. and move intermittently throughout the day Preferred Qualifications Bachelor's degree in social work or a related human service field Experience working with geriatric populations or long-term care settings Knowledge of state and federal regulations governing admissions to a nursing facility Certification in social work (LCSW) or a related field What We Offer Competitive salary and benefits package Opportunities for professional growth and development Collaborative and supportive work environment Recognition and rewards for outstanding performance A sense of purpose and fulfillment from knowing you're making a difference in the lives of others How to Apply If you're a motivated and compassionate individual who is passionate about delivering exceptional care and support to our residents, we encourage you to apply for this rewarding opportunity.
