In this role, you will be responsible for evaluating, diagnosing, and treating patients with speech, language, cognitive-communication, and swallowing disorders. Typical responsibilities include completing thorough evaluations, developing individualized plans of care, delivering evidence-based therapy, and documenting progress in the EMR. You will provide dysphagia assessment and treatment, conduct communication and cognitive-communication interventions, and offer patient and family education to promote carryover of strategies and safe discharge planning. Participation in interdisciplinary care conferences and regular collaboration with nursing and other rehab disciplines is a core part of the position. A typical day includes a mix of evaluations and treatment sessions with primarily adult and geriatric patients in a skilled nursing and subacute rehab environment. Caseloads are designed to be manageable and clinically focused, with patient care tailored to functional outcomes such as safe swallowing, improved communication, and cognitive support for daily activities. You can expect exposure to a wide range of diagnoses, including stroke, progressive neurological conditions, dementia, cardiopulmonary conditions, and post-surgical or medically complex cases. The setting offers meaningful opportunities to develop and refine skills in geriatric communication and dysphagia management.
