Summary This position is a Social Worker with duty station located at the Sisseton Service Unit, Sisseton, South Dakota. This position reports to the Behavioral Health Supervisor or Designee. Responsibilities Assess and evaluate patients using a patient centered medical home approach using therapeutic techniques to diagnose and treat mental health diagnosis. Develops interventions based on above need assessment and exercises mature professional judgment in consultation with the supervisory social worker in providing therapeutic modalities that are culturally appropriate. Participates and when appropriate takes a leadership role in assessing community human service needs. Works with community members and human resources -providers to develop approaches to addressing the needs of patients. Assists in monitoring and reviewing services for quality and appropriateness of care. Assists in developing as needed appropriate services to address patient and family psychosocial needs. Background Investigation Requirement: This position requires the successful completion of a federal background investigation as a condition of employment. Must fulfill all of the requirements for the master's degree in social work in a school accredited by the Council on Social Work Education. Master's degree with practicum: Applicants who complete all of the requirements for a master's degree in social work that included a practicum are eligible for GS-9. In addition to the Basic Requirements, you must also meet the Minimum Qualifications stated below. MINIMUM QUALIFICATIONS: Your resume must demonstrate at least one (1) year of specialized experience equivalent to at least the next lower grade level in the Federal service obtained in either the private or public sector performing the following type of work and/or tasks: GS-11: 1 year of Specialized Experience equivalent to at least GS-9 grade level. OR 3 years of progressively higher-level graduate education leading to a Ph.D. degree or Ph.D. or equivalent doctoral degree. Specialized Experience Defined: GS-11: Collaborate with other health professionals to assess client needs; refer patients to community resources; provide individual or group counseling; coordinate care with expert diagnostic and treatment consultation to medical providers; assess in-patient needs and patient discharge planning.
