Work Environment\nThis role is primarily remote, which allows for flexibility in providing client support and assessments while still maintaining essential face-to-face interactions as needed. Regular hours are Monday to Friday from 8:00 AM to 4:30 PM, with occasional evening or weekend hours based on client needs. You will work closely with our interdisciplinary team, including nurses, physicians, and caregivers, ensuring a holistic approach to each client's care plan.
Key Responsibilities\n- \n
- Conduct comprehensive psychosocial assessments to determine the needs of seniors and their families, focusing on emotional, mental, and social well-being. \n
- Develop personalized care plans in collaboration with clients and their families, setting specific goals and addressing individual preferences and priorities. \n
- Provide counseling and emotional support to clients, guiding them through end-of-life issues and the associated challenges. \n
- Advocate for clients’ rights by facilitating access to necessary services, community resources, and support systems. \n
- Engage with an interdisciplinary team to coordinate care seamlessly, ensuring that the physical, emotional, and social aspects of each client’s needs are met. \n
- Conduct regular home visits to monitor client progress, manage care adjustments, and assess living conditions to ensure safety and comfort. \n
- Help families communicate with one another, addressing potential conflicts surrounding caregiving responsibilities and emotional support. \n
- Educate clients and families about resources, support programs, and community services available to enhance overall quality of life. \n
- Maintain accurate documentation of evaluations, care plans, and progress notes in compliance with health regulations and facility policies. \n
- Stay current with professional standards, regulatory updates, and social work ethics to maintain quality care within the hospice setting. \n
The primary client population consists of seniors and their families dealing with end-of-life challenges in a hospice setting. Qualifications\n- \n
- Must hold a master's degree in Social Work from an accredited institution. \n
