Job Description
Moffett Training Plan | R.A.S. Logistics · October 13, 2026 · Nick Kochek
Purpose and scope
No one operates a Moffett for R.A.S. Logistics until they have completed all five phases of this plan and hold a signed certification from a qualified evaluator. The plan meets OSHA's powered industrial truck standard, 29 CFR 1910.178(l), which requires formal instruction, hands-on training and an evaluation of the operator in the actual workplace.
- Who it covers: every driver, helper, ISP team member or manager who will mount, dismount or operate a truck-mounted forklift (Moffett or similar) on an R.A.S. route or terminal.
- Equipment: truck-mounted forklifts are rough-terrain forklifts under OSHA. Training is specific to the model the person will run. A move to a different model means retraining on that model before using it.
- Minimum age: 18. Federal child labor rules bar anyone under 18 from operating a forklift.
- What the plan does not cover: CDL or DOT driver qualification for the carrying truck. That is handled separately.
Roles and responsibilities
- The terminal manager owns the result. The certified trainer does the training. The trainee does not operate alone until the paperwork is signed.
- Role Responsible for Terminal manager Names a certified trainer for each terminal. Schedules trainees. Keeps the certification file current. Pulls anyone off the equipment after an incident.
- Certified trainer / evaluator Has the knowledge, training and experience to train and evaluate (OSHA's standard). Teaches all five phases. Signs off only after watching the trainee meet every pass standard.
- Trainee Completes every phase. Runs the pre-use inspection every shift. Reports any defect, near miss or damage the same day.
- Carrier partner / ISP owner Sends only trained, certified people to run Moffett routes. Gives the terminal a copy of each certification.
Training phases
Training takes 4 hours in total, all in one session at the terminal. A trainee moves to the next phase only after passing the gate for the current one. If they miss a gate, they get another session; the 4 hours are never cut short.
Phase Time What's covered Gate to move on
- Classroom 60 min Operator's manual for this model. Data plate and load chart. Stability and center of gravity. R.A.S. operating rules. Written test (last 15 min). Pass the written test
- Walkaround and inspection 30 min Every control, gauge and safety device. Full pre-use inspection using the checklist. Inspection done with no prompting
- Mount and dismount 30 min Truck setup, dismount, mount, and fastening every tie-down. Two clean cycles
- Yard practical 90 min Coned course: forward and reverse, tight turns, a slope, gravel or grass, picking and placing real product at height. Course run clean twice in a row
- Evaluation and sign- off 30 min A mock delivery stop in the yard, with the trainer playing the customer, scored on the skills checklist. All critical items pass, certification signed Total 4 hours
Daily pre-use inspection and mount/dismount
The operator inspects the unit before the first use of every shift and signs the checklist. Any failed item takes the unit out of service until it is repaired. OSHA requires the check at least daily (1910.178(q)(7)).
Engine off
- Fluid levels: engine oil, hydraulic oil, coolant, fuel
- Hydraulic hoses, cylinders and fittings: no leaks, cracks or rubbing
- Forks: no cracks or bends, heel not worn, locking pins in place
- Mast, chains and carriage: no damage, chains evenly tensioned
- Tires: pressure, cuts, missing lugs
- Data plate and decals present and readable
- Seat belt and operator restraint working
- Mounting kit on the truck: pockets, brackets, tie-down chains and binders in good shape Engine running
- Horn, back-up alarm, lights and beacon
- Service brake and parking brake hold
- Steering and all drive modes
- Lift, tilt, reach and side-shift run smooth with no drift
- Gauges and warning lights normal
Mount and dismount
- Park the truck on firm, level ground with the parking brake set and hazards on. Set cones.
- Check the area behind the truck for people, vehicles and overhead lines before moving the unit.
- Dismount: release the tie-downs, then lower the unit off the mounting kit using the models procedure. Keep the forks low and clear of the truck.
- Mount: line the unit up square to the truck, raise it slowly into the mounting pockets, and confirm both sides are seated.
- Set the parking brake, shut the unit off, and fasten every tie-down chain and binder. Walk around the truck to check before pulling out.
Safe operating rules
These rules apply on every stop. Breaking a critical rule (marked ★) removes the operator from the equipment until they are retrained.
Load handling
- Never go over the rated capacity on the data plate. Remember that capacity drops as the load is reached out or raised.
- Spread the forks as wide as the load allows. Keep the load tilted back against the backrest while traveling.
- Travel with the load low, about 6 to 12 inches off the ground. Never travel with it raised.
- No riders on the forks or the unit, and never lift a person.
Terrain and travel
- Walk the path before you drive it. Look for soft ground, curbs, slopes, holes, septic and sprinkler lines.
- On a slope, keep the load facing uphill and never turn across the slope.
- Wear the seat belt at all times. If the unit starts to tip, stay in the seat, hold on and lean away from the fall. Do not jump.
- Slow down on gravel, grass, wet or icy surfaces, and over any bump.
People and customer sites
- Nobody stands or walks under a raised load or within the work zone. Stop if someone enters it.
- Make eye contact with the customer and explain where the unit will move before starting.
- Look behind before every reverse move. Use a spotter in tight or blind areas.
- Keep at least 10 feet from overhead power lines.
- Never drive onto a surface that could be damaged (a driveway, lawn or deck) without the customer's OK. Note any existing damage in a photo first.
Leaving the unit
Lower the forks flat to the ground, set to neutral, set the parking brake and shut off the engine
whenever you walk away.
Evaluation and certification
- To certify, a trainee must score at least 80% on the written test and pass every critical skill item on the practical evaluation. Missing any critical item means more yard practice, then a full re-evaluation.
- Written test: 20 questions covering the data plate and load chart, stability, the pre-use inspection, the operating rules, and what to do in a tip-over. Pass mark is 16 of 20. Go over every missed question with
- the trainee before they move on.
- Skills checklist (mock delivery stop)
- Skill Critical Pass / Fail
- Completes pre-use inspection and checklist Yes
- Sets up the truck: level ground, brake, cones Yes
- Dismounts the unit safely Yes
- Walks and checks the path before driving it Yes
- Wears seat belt the whole time Yes
- Confirms the load is under rated capacity Yes
- Travels with the load low and tilted back Yes
- Handles slopes correctly (load uphill, no turning across) Yes
- Keeps people clear of the work zone Yes
- Checks behind before every reverse move Yes
- Places the load where the customer asked, with no damage No
- Communicates clearly with the customer No
- Parks correctly: forks down, brake set, engine off Yes
- Mounts the unit and fastens every tie-down Yes
The certification record must include, as OSHA requires: the operator's name, the training date, the evaluation date, and the name of the trainer and evaluator. Add the equipment model, written test score, and both the operator's and evaluator's signatures.
- Operator name
- Equipment model
- Training date
- Evaluation date
- Written test score / 20
- Trainer / evaluator name
- Operator signature
- Evaluator signature
- Refresher, retraining and records
Every operator gets a live-route re-evaluation at least once every 3 years, as OSHA requires. R.A.S. adds a yearly check-ride on top of that. Retraining is required before the next shift when any of these happen (1910.178(l)(4)):
- The operator is seen running the unit unsafely, or breaks a ★ rule.
- There is an accident or a near miss: a tip, a dropped load, or damage to property or the truck.
- An evaluation shows the operator is not running the unit safely.
- The operator is assigned to a different model.
- Work conditions change in a way that affects safe operation, such as a new type of product or a new site type.
Records
- Each terminal keeps a certification file per operator, plus the signed daily inspection checklists. Keep checklists for at least 12 months.
- The terminal manager reviews the file every month and schedules re-evaluations before they come due.
- Incident reports go into the operator's file with the retraining that followed.
