Seven Healthcare are seeking a dedicated Physical Therapist for a contract-to-hire assignment in Upper Marlboro, MD. This role is perfect for someone passionate about providing high-quality therapy services in both Prince Georges County and Montgomery County. The successful candidate will work onsite, engaging with patients to enhance their physical independence and health outcomes through detailed evaluations and tailored treatment plans.

Key Responsibilities of the Physical Therapist
- Perform physical therapy evaluation and treatment of patients based on physician orders/referrals
- Complete comprehensive evaluations and assessments using appropriate tools and procedures
- Review patient records for accurate diagnosis and treatment planning
- Establish measurable goals and develop individualized treatment plans
- Regularly reassess patient progress and adjust treatment plans as necessary
- Communicate patients' needs and progress to the treatment team and relevant parties
- Collaborate with medical providers regarding patient management and discharge planning
- Recommend and assist in the acquisition of adaptive equipment for patients
- Maintain and submit necessary documentation as per requirements
- Supervise Physical Therapist Assistants and Aides
- Participate in staff development programs and maintain clinical competencies and certifications
Requirements of the Physical Therapist
- Compact State or Maryland State Licensed Physical Therapist
- Experience with Oasis documentation and Homecare Homebase EMR system
- Strong customer service and time management skills
