Overview

Schedule: Monday-Friday 7:30am - 4:00pm, 8hr shifts.

Purpose Statement/Position Summary: The Community Health Worker II, under general supervision of department leadership, provides specific services to patients and their families. Coordinates case management and social care services. Demonstrates commitment to the principles of family-centered care. Serves as the first level of support to community health worker team members for resource and training questions.

Minimum Qualifications/Work Experience:

Required : 2 or more years of experience as a community health worker, parent partner, navigator, case manager, or similar position. 1 or more years of experience with community resource navigation, social-care support, and/or health education.

Preferred : Experience training team members and facilitating presentations, preferred. Experience with public/government benefits through work or lived experiences, preferred. Bilingual skills, preferred.

Education/Licensure/Certification:

Required : High school diploma/GED OR 4 years of related education and/or work experience.

Pay Scale Information

$52,728.00-$70,304.00

The pay scale listed for this position is generally for candidates who meet the specified qualifications and requirements listed on this specific job description. Additional pay may be determined for those candidates who exceed these specified qualifications and requirements.
