You will report to our Associate Medical Director.Why You'll Love Working HereTeam-Based Care Model: Partner with a dedicated care team that supports patients' physical, emotional, and social wellness.More Time with Patients: See fewer patients per day and spend more time delivering meaningful care.Supportive Culture: We foster a welcoming, inclusive environment where teamwork and growth are prioritized.Your Role & ResponsibilitiesYou will provide comprehensive care to seniors following clinical standards.You will collaborate daily with your care team to ensure coordinated, high-quality care.You will manage referrals, hospital/SNF coordination, DME, and home health services.You will document care accurately and efficiently with support from quality-based coders.You will participate in on-call rotation and support growth plans at your center.You will have passion for providing value-based care.What We're Looking ForRequired:Master's Degree in Nursing or completion of a PA program with board certification.Active, unrestricted NP or PA license in the state of practice.If selected for this role, you will be required to be screened for TBPreferred:Experience in value-based care, primary care or Medicare Advantage settings.Active DEA license and Medicare/Medicaid provider numbers or ability to obtain DEA license and/or Medicaid and Medicare numbersFamiliarity with HEDIS quality indicators and Medicare guidelines.Location:Conviva Royal Palm Beach Center in FloridaUse Your Skills to Make an ImpactLanguage Proficiency Testing: any associate who speaks with a member in a language other than English must take a language proficiency assessment, provided by an outside vendor, to ensure competency. Applicants will be required to take the Interagency Language Rating (ILR) test as provided by the Federal Government.Scheduled Weekly Hours40Pay RangeThe compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.$123,800 - $170,300 per yearThis job is eligible for a bonus incentive plan.
