- Possession of, or Ability to attain active, unrestricted CA license
- Board Certified or Truly Board Eligible
- Unrestricted DEA
- Clean Background/DEA/NPDB for past 7 years
- Supervision of Mid-Level providers
- Ability to work as the solo Physician
- AHA Certifications: ACLS, BLS, NRP
