Job Summary:

Key Responsibilities:
Provide social work services to residents, including assessment, counseling, and psychotherapy
Develop, implement, and evaluate individualized care plans to address social, emotional, and psychological needs
Collaborate with residents, families, and healthcare teams to ensure comprehensive care
Participate in interdisciplinary team meetings to discuss resident care and progress
Document resident interactions, assessments, and treatment plans accurately and in a timely manner

Requirements:
Licensed Social Worker (LSW) certification in the state of Michigan
