What You\'ll Do As a Home Health Physical Therapist, you\'ll: Conduct initial patient assessments in the home setting Develop and implement individualized treatment plans Coordinate care with physicians and healthcare teams Promote mobility, independence, and healing through compassionate, hands-on care Use our proven case management model to guide patient outcomes and support families What You\'ll Need Active Florida Physical Therapist license Minimum 2 years of post-graduate experience preferred Orthopedic or home health experience is a plus Passion for patient-centered care and commitment to quality outcomes Locations We\'re hiring in multiple counties across Florida.
