Shift Days Schedule 8:00am-8:30pm Job Details Cleveland Clinic’s Business Operations Center serves as a key hub for our Home Care and Hospice operations as well as many administrative and support teams. This collaborative workplace supports a wide range of caregivers, clinical professionals, and operational teams. While many positions are based within the center, numerous home-visiting roles—including case managers, nurses, and therapists—travel throughout the Greater Cleveland and Akron/Canton areas to deliver exceptional care directly to patients in their homes. Keeping patients connected to the highest quality care as theytransitionfrom the hospital to home orafacility is part ofthe Cleveland Clinicinnovative approach to value-based care.As a Hospice Coordinator,you will be at theirside the whole way, ensuring a smooth adjustment to their new routine. Through our Center for Connected Care,you will helpprovide a full continuum of careby processing inpatient referrals, coordinating discharge plans, coordinating services and more.Your efforts directly enhance the quality of life for patients and their families every day. A caregiver in this position works3-10s, 8:00am to 6:30pm, including every 4th weekend and 2 holidays per year (1 winter, 1 summer). This role will provide coverage to our west and south region hospitals. A caregiver who excels in this role will: Act as a member of the interdisciplinary team for the assigned functions related to the coordination of services with internal and external referral sources. Processinpatient referrals into hospice, ensurecontinuity of care and coordinatedischarge planning in contract facilities. Actively educate potential customersaboutthe benefits and appropriateness of hospice care. Ensure patientsand their familieshaveadequate knowledge and an understanding of the hospice benefit. Process inquiries for all new referrals (internal and external). Ensure a seamless transition from hospital to the community setting byorderingmedical supplies and DME,handingoffcommunication with the field team and documentinginthe electronic medical record. Perform assessment of potential patients consistent with Cleveland Clinic Hospice policies,proceduresand admission criteriaalongside thephysician. Perform admission assessments for new admissions into contracted facilities and document information in the EMR withinspecifiedtime frame. Planthedelivery of patient care in collaboration with other disciplines to provide comfort and symptom management. Perform daily assessment and coordinatethe plan of care withinpatient staff for General Inpatients (GIP). Assesspatientfor safety risks,abuseand neglect. Assess and document pain usingtheappropriate painscale and managepatient’spain. Documentchangesin patient condition and effectiveness/timeliness of care/interventions. Evaluate,updateand revisetheplan of caretofacilitateachievement of expected outcomes. Know action, usual dosage,indicationsand side effects of medications administered. Attendinterdisciplinary meetings as assigned. Minimum qualifications for the ideal future caregiver include: Graduate from an accredited school ofProfessionalNursing Current state licensure as a Registered Nurse (RN) One year of current clinic experience as an RN One year of experience in an acute med/surg or critical care setting Basic Life Support (BLS)Certification through the American Heart Association (AHA)or the American Red Cross Knowledge of JCAHO and Medicare standards and regulations Ability to work irregular/varying hours as needed Ability to travel to hospitals and community referral sources Preferred qualifications for the ideal future caregiver include: Bachelor’sofScience inNursing (BSN) Physical Requirements : Requires full range of body motion including handling and lifting packages, manual and finger dexterity and eye hand coordination. Occasionally lifts and carries items up to 50 pounds. Requires corrected vision and hearing to normal range. Requires standing and walking for extensive periods of time.
