RN Case Manager: Care Coordination & Transition Lead

The inpatient RN Case Manager coordinates patient care and resources from admission through hospital stay and transition to the community. This role requires strong communication, care coordination, and adherence to guidelines for Medicare/Medi-Cal beneficiaries. The position involves 0800-1630 shifts, a 36-hour workweek, and day-to-day scheduling patterns such as Sunday to Wednesday (week 1) and Sunday-Thursday (week 2).
