Qualifications:
• MD or DO degree
• Active U.S. medical license
• Experience in Family Medicine, Internal Medicine, Gerontology, Palliative Care, or Home Care
• Strong background in adult primary care and chronic disease management
• Interest or experience in value-based care preferred
• Excellent communication and interpersonal skills
