Full Time Position Available Monday-Friday 8:00am-4:30pm
Successful Candidates:
- High Schoolgraduate or GED required
- KansasCertified Nurse Aide Certification and Kansas Home Health Aide Certification
- CurrentBLS and driver's license
- Becustomer-oriented and possess the ability to work independently with minimal supervision
Essential Functions:
- Perform basis assessment of patients and provide services that are included in the patient's care plan
- Report measurable outcomes as requested and communicate and changes in the patient's medical, nursing, rehabilitative, and social needs to the hospice nurse
- Provide support and education to the patient/family and/or caregiver about the patient's disease process
