Advanced Care at Home (ACH) is an innovative care model within the Division of Hospital Internal Medicine at Mayo Clinic Arizona that delivers comprehensive, hospital-level care to patients in the comfort of their homes through a combination of advanced technology, virtual care, and in-person clinical services.
The Nurse Practitioner/Physician Assistant (NP PA) plays a vital role in transforming healthcare delivery by providing safe, high-quality, patient-centered care across the continuum. The NP PA works collaboratively with physicians, nurses, pharmacists, case managers, command center operations, and community-based care teams to deliver exceptional clinical outcomes and patient experiences.
As a key member of an emerging and evolving model of care, the NP PA serves as both a clinical expert and ambassador for hospital-at-home medicine. This role requires the ability to champion the Advanced Care at Home program within the hospital, educate patients and colleagues about the benefits of hospital-level care delivered in the home, and convey genuine enthusiasm for this innovative approach to care delivery. Key Responsibilities
- The APP independently and collaboratively manages patients receiving hospital-level care in the home setting through responsibilities that include:
- Obtaining comprehensive patient histories and performing physical examinations
- Formulating diagnostic impressions and developing evidence-based treatment plans
- Ordering, interpreting, and performing diagnostic and therapeutic procedures within scope of practice
- Monitoring patient progress and evaluating the effectiveness of therapeutic interventions
- Coordinating consultations, referrals, and transitions of care
- Providing education, counseling, and support to patients and their families
- Collaborating closely with interdisciplinary care teams to ensure seamless, high-quality care delivery
- Maintaining prescribing authority and clinical privileges as required by Arizona regulatory and institutional standards
- Serving as an advocate for quality, safety, service excellence, and continuous improvement within the ACH program
- Promoting awareness and understanding of the ACH model among patients, families, and healthcare colleagues

Qualifications Acute Care Nurse Practitioner:
Masters-prepared graduate of an accredited school of nursing (ANCC) and Nurse Practitioner program. Current AZ RN and APRN license and certification as a ACNP.
Physician Assistant:
Graduate of accredited Physician Assistant program with current PA certification by NCCPA and registration as a PA by the AZ State Board of Medical Practice.
Additional Qualifications:
- At least one year of hospital-based experience is required.
- The NP PA must be flexible and willing to work 12-hour shifts, weekends, and holidays.
- Experience providing care in patient’s homes or via telehealth/remote monitoring is a preferred.
- Active BLS and ACLS certification from American Heart Association or American Red Cross at start date .
- Evidence of application for DEA certificate must be provided upon hire and obtained within 90 days of hire. Certification should be maintained thereafter unless waived by department/division.
- A valid driver’s license and an acceptable driving record per an annual motor vehicle record check are required
- The NP PA must have excellent verbal and written communication skills and possess strong computer skills.
