RN Hospice Case Manager

Schedule : Mon - Fri 8am-430pm, 40 hrs/wk. No call.

Required : Active/clear CT RN license (or compact),

• Associates or Bachelors degree.
• Prior hospice nursing experience
• Experience with Homecare Homebase (HCHB)
• Strong case management and interdisciplinary team collaboration skills
• Ability to manage a hospice caseload independently while providing compassionate, patient-centered care.
