Residential Case Manager

Position Title: Residential Case Manager
Program(s) and Location: Residential (Sacramento, CA)
Schedule: Full-time, Non-Exempt, 40 hours per week, Tuesday through Saturday 8am to 4:30pm
Pay Rate: $23.05 hourly
About the Position:
Under the direct supervision of the designated individual on the agency Organizational Chart, the Case Manager works as part of a team to provide and coordinate services and resources for clients transitioning from homelessness into permanent supportive housing. The goal of the Case Manager is to provide referrals, crisis intervention, support, and encouragement to enable clients to access and use services that prevent hospitalization and incarceration, maintain permanent housing, and improve their quality of life. This position assists clients with financial matters and entitlement issues. It provides the care and attention needed to enable clients to develop better coping skills, independent living skills, and techniques that empower them with recovery, a sense of responsibility, and independence.
Essential Duties & Responsibilities:
- Support and Counseling: Support individuals with active listening and empathy and convey a message of hope and recovery. Observe, assess, intervene, and refer (within one's scope of practice) in situations involving clients' mental health symptoms, physical health symptoms, and substance use issues.
- Service Planning and Goal Setting: Develop service/goal plans with clients and meet with them as needed to encourage, support, and advocate for progress and skill development.
- Housing Support: As needed, monitor and support the housing application process. When necessary, assist clients with transportation in personal vehicles.
- Team Collaboration: Work with a multi-disciplinary team to provide integrated psychosocial rehabilitative services to clients in the community and their homes. Attend meetings to ensure continuity of care for residents.
- Public Relations and Advocacy: Provide positive public relations, community referrals, information, and support by linking clients to needed resources. Advocate with and on behalf of clients and their identified significant support persons.
- Documentation: Accurately complete all necessary forms, reports, and paperwork required by the program on a timely basis (e.g., progress notes, special incident reports, discharge summaries, HMIS).
- Support Activity Facilitation: When applicable, support the Residential Service Coordinator in facilitating the planning of social, cultural, educational, supportive, and recreational group activities.
- Life Skills Coaching: Shop for food and household supplies to meet program or client needs. Ensure the living space is clean and well-kept. Provide coaching and assist with cleaning and personal hygiene as needed.
- Mandated Reporting: Act as a designated Mandated Reporter for the observed or suspected abuse and neglect of children, disabled individuals, dependent adults, and elder/senior citizens. Include "Duty to Warn" responsibilities, including notifying authorities and intended victim(s) of physical harm.
- HIPAA Compliance: Follow all policies and procedures in the agency HIPAA Manual to ensure the protection and confidentiality of all Protected Health Information (PHI).
- File Management: Ensure up-to-date tenant files are maintained per agency standards.
- Other duties as assigned.
Qualifications:
- Ability to work with individuals who have a mental health diagnosis. Experience is highly desirable.
- Experience with persons who have substance abuse problems, preferably individuals who are dually diagnosed (mental health & substance abuse), is highly desirable.
- Knowledge of 12-step Programs and personal experience with substance abuse recovery is highly desirable.
- Lived experience as a mental health consumer or a family member/loved one of a mental health consumer is desirable.
- Mental health crisis intervention experience is highly desirable.
- Experience working with children, adolescents, and families is required. Background in childhood education, youth services, family support, or a related field is highly desirable.
Certificates, Licenses, Registrations:
- Possess a valid California Driver's License and current vehicle insurance. Driving records must meet, and continue to meet, the underwriting standards established by the generalized driving guidelines used by our insurance broker. Must provide proof of insurance.
- Must have at least 3 or more years of driving experience.
Other Skills, Abilities, and Job Requirements:
- Ability to pass clearance of the Federal Bureau of Investigation (FBI) and/or Department of Justice (DOJ) background screenings
- Consumer of mental health services or a family member of a mental health consumer is desirable
- Work overtime as requested and approved, including after-hours duty as needed
