Pediatrics Nurse Case Manager needed in Chicago, IL

Overview HealthPlus Staffing is seeking a Nurse Case Manager in Chicago, IL. The Nurse Case Manager will provide help to high risk infants and children getting health care and developmental services. The Nurse Case Manager will be responsible for maintaining a caseload of approximately 100 active clients by conducting outreach activities and utilizing the referral system from High Risk Infant follow Up Program from the State of Illinois.
