Responsibilities Provide 1:1 care following the client’s plan of care, maintaining dignity and independence. Administer medications, feedings, oxygen, and ostomy care as needed and assess results. Accurately document nursing actions and communicate progress and outcomes with families, case managers, and physicians. Coordinate or order supplies, medications, and feedings to ensure adequate amounts are on hand. Ensure all patient needs are met in accordance with physician instructions. Qualifications LPN/LPT/LVN degree from an accredited nursing college with current unrestricted registration and license in the state of practice and residence. Valid driver’s license, acceptable driving record, and proof of car insurance. CPR professional certification. 1+ year nursing experience preferred. Home health care experience preferred.
