Street Medicine Operations Specialist

The Operations Specialist for Street Medicine is responsible for the daily operations of the Epstein Street Medicine Program and clinical services through Project HOME Healthcare Services (PHHS). The Operations Specialist for Street Medicine will work closely with the Clinical Lead for Street Medicine, Street Medicine team, Project HOME stakeholders, and City and Social Service partners in homeless services to address the complex health services needs of individuals experiencing homelessness/housing instability in Philadelphia. They are responsible for all administrative duties needed to optimize patient engagement, patient care, improve processes, and increase patient and staff satisfaction through holistic, trauma-informed and patient-centered care delivery. What You Will Do: Engage with street medicine participants who are looking to access healthcare services. Create and communicate weekly schedules for clinical team members in collaboration with Street Medicine Clinical Lead, Mobile Clinical Supervisor, Project HOME Homeless Outreach based on patient acuity, staffing availability and location needs. Create an on-call schedule for after-hours patient calls. Gather and communicate daily hospital reports for hospitalized Street Medicine patients. Primary contact for: urgent safety issues, referral coordination, scheduling, follow-ups & chart updates. Manage the day-to-day operating budget for Street Medicine, including expense tracking, monitoring, and reporting. Participate in the development of the annual operating budget for medical services, including jointly determining programmatic and staffing needs in partnership with the Street Medicine Clinical Lead and the Clinical and Operations Directors. Coordinate and participate in regular clinical rounds with Street Medicine Team (including but not limited to providers, nurse care coordinators, and behavioral health consultants) to support clinical decision making. Design, evaluate and implement medical workflows and processes in collaboration with Street Medicine Clinical Lead. Order and track operational and medical supplies, including durable medical equipment (DME), vaccines and injections, as needed to support street medicine functions in coordination with Street Medicine Clinical Lead and Nurse Care Coordinator. Prepare data and narrative reports and represent the Street Medicine Program as required for varied governmental and nongovernmental funding sources with the support of the Clinical Lead for Street Medicine. Represent PHHS and Street Medicine in interagency meetings in collaboration with Clinical Lead. Collaborate with outreach, shelters, community partners, and city agencies as needed to ensure continuity of care. Drive outreach van as needed/transport PH street medicine staff and patients. Act as the Vehicle Point Person (VPP) for the street medicine vehicle. Participate and engage in hospital visits for patients that have been admitted, when available. General Responsibilities Manage all administrative day-to-day activities for clinical care, including real-time provider schedule management. Perform patient engagement, intake, scheduling, registration, insurance enrollment and verification, and service delivery. Serve as the primary liaison between the clinical teams and administration, facilitating effective communication and implementation of organizational initiatives. Answer phone calls, address patient inquiries, and provide information regarding Street Medicine and PHHS. Quality Improvement Support reaching annual goals for Street Medicine Program that align with and support Project HOME's mission. Support quality improvement project(s) for Street Medicine to improve patient care and safety. Develop and maintain activities to achieve Patient Centered Medical Home status. Perform other duties as assigned or requested. What You Will Need A high school diploma or GED. At least 3 years' experience working with unhoused individuals and/or people who use substances or have mental health diagnoses. Must possess and maintain a valid driver's license and be eligible to drive on behalf of the organization. Candidates must meet the organization's vehicle insurance eligibility requirements, including a driving record with no major motor vehicle violations and only minimal minor traffic violations, as determined through a motor vehicle record (MVR) review. What You May Bring CPR and First Aid certification preferred. Experience in current or previous work environments that involved healthcare operations, community-based services, social services, outreach, patient navigation, care coordination, or administrative support. Experience working in team-oriented, or public-facing environments where strong follow-through, coordination, problem-solving, and communication were needed. Strong organizational skills, with the ability to manage multiple projects, meet deadlines, and prioritize tasks effectively. Excellent verbal and written communication skills. Strong interpersonal skills, with the ability to collaborate across departments, locations, and levels of staff while remaining calm under pressure.
