Registered Nurse (RN)

Job Description

We are seeking an experienced Registered Nurse (RN) to join our home health team. The ideal candidate must possess a valid nursing license and have prior experience in home health care and home care operations.

Responsibilities

Manage a caseload of approximately 30 adult patients.
Complete start of care assessments, recertifications, and daily visits.
Provide high-quality home health services with a focus on patient well-being.
Collaborate with the clinical manager or educator for support and guidance as needed.

Qualifications

Current, active RN license in the state of practice.
Minimum of 1 year experience on a Medsurg floor in a hospital setting.

Work Schedule

Typical shifts are 5 shifts of 8 hours each.
Weekend shifts may be scheduled based on team rotation; hours are generally 8 AM to 5 PM.
Participation in holiday rotation is required.

Orientation

New hires will undergo a two-week orientation period, which includes several visits with a work coach.

Equipment

A phone and tablet will be provided for necessary communications and documentation.

Location

This position operates within a specified city and state, fostering a home health environment focused on patient-centered care.
