Infusion Homecare Registered Nurse (RN) Per-Diem - Canandaigua/Ontario County

Role Overview We seek home infusion field nurses (registered nurses) to provide high-quality home-based services in Canandaigua/Ontario County and surrounding areas. Responsibilities Conduct patient visits to initiate care, perform follow-up visits, and precept and evaluate newly hired nurses. Partner with the Infusion Intake Team to ensure seamless initiation of infusion care in patients' homes. Collaborate with the infusion manager, coordinators, and other office staff. Conduct on-site field visits to provide patient care and supervise nursing staff competence as requested. Multitask, prioritize work responsibilities, and manage time effectively. Ensure patient safety, monitor responses, perform post-infusion follow-up. Communicate with patients, families, infusion pharmacy, and medical providers as necessary. Maintain paperless clinical documentation using a computer and tablet. Perform comprehensive nursing assessment, PIV, PICC, mid & central line care, and access competence. Competence in implanted port access (e.g., MediPort, PermaCath) is a plus. Qualifications At least 3 years of full‑time or equivalent part‑time experience as a registered nurse in a home care infusion company, hospital, clinic, or medical facility. Supervisory or managerial experience preferred. Infusion experience required – must infuse IV/SQIG and biologic medications. Knowledge of supervision, performance evaluation, and education practices. Knowledge of Federal HIPAA regulations. Knowledge of standard nursing policies, procedures, methods, and techniques. Ability to read and interpret clinical documents, including physicians' orders and treatment plans. Experience participating in a multidisciplinary health care team. Job Requirements NYS Registered Nursing License and Registration Certificate. BLS necessary; ACLS preferred. Up-to-date medical, including Physical exam, TB clearance (QuantiFERON Gold), 2-step PPD or T-Spot, or Chest X-ray, and habituation statement. MMR/Varicella titers lab report or vaccination records. TDAP and Hep B vaccination series, titers OR declination. Urine Drug Screen (UDS) within the last 6 months acceptable. Flu vaccination (if applicable) or declination. Two (2) forms of government-approved ID to validate employment eligibility. Two (2) satisfactory professional references. Malpractice insurance. Reliable transportation. Pay and Schedule Pay: $50.00+ per hour (base); cases pay $60–$150+ based on the level of care provided. Minimum hours: 10 hours per week (home visit cases).
