Spring River Rehabilitation & Care Center is seeking an experienced, motivated, and resident-focused Assistant Director of Nursing (ADON) to join our clinical leadership team.
The ADON is a key nursing leader responsible for assisting the Director of Nursing (DON) with the planning, coordination, implementation, and evaluation of nursing services throughout the facility. This position provides daily clinical leadership while supporting nursing operations, staffing, regulatory compliance, employee development, quality improvement, and resident-centered care.
The ADON should be a highly visible and engaged leader who is comfortable working directly with nursing staff, residents, families, providers, and members of the interdisciplinary team. The position requires a balance of clinical expertise, leadership, accountability, problem-solving, and staff development .
Key Responsibilities Clinical Leadership & Nursing OperationsAssist the Director of Nursing with the daily management and oversight of nursing services
Provide visible clinical leadership throughout the facility and regularly round on nursing units
Monitor resident care and nursing practices to ensure established standards of care are maintained
Support consistent nursing operations across shifts, units, weekends, and changing resident needs
Serve as a clinical resource for RNs, LPNs, CNAs, and other nursing personnel
Assist nursing staff with complex resident-care situations and changes in condition
Promote timely communication with physicians/providers, residents, families, and interdisciplinary team members
Participate in admissions, transfers, discharges, and significant changes in condition as appropriate
Assist with emergency response and clinical decision-making when necessary
Assume nursing leadership responsibilities in the absence of the Director of Nursing as assigned
Assist with evaluating nursing staffing levels based on resident census, acuity, and facility needs
Work collaboratively with nursing leadership and scheduling personnel to maintain appropriate coverage
Identify staffing gaps and assist with developing timely solutions
Support efforts to minimize unnecessary overtime and agency utilization while maintaining appropriate resident care
Assist with interviewing, selecting, onboarding, and retaining nursing employees
Participate in nursing orientation and ensure new employees receive appropriate clinical support
Provide coaching, counseling, and performance feedback to nursing staff
Assist with attendance, performance, and accountability concerns in collaboration with the DON and Human Resources
Promote employee engagement, teamwork, accountability, and retention within the nursing department
Monitor resident outcomes and identify opportunities for clinical improvement
Review changes in condition, hospital transfers, falls, wounds, infections, weight changes, medication concerns, and other clinical indicators as appropriate
Assist with identifying residents at increased risk and ensure appropriate interventions are implemented
Participate in clinical meetings and resident risk reviews
Assist with root-cause analysis and development of corrective interventions following adverse outcomes
Monitor implementation and effectiveness of resident care plans
Promote evidence-based clinical practices and resident-centered care
Work collaboratively with nursing, therapy, dietary, social services, MDS, pharmacy, and medical providers to improve resident outcomes
Conduct routine clinical audits to evaluate documentation accuracy, completeness, and compliance
Review nursing documentation, medication and treatment records, physician/provider orders, assessments, and care-related records as assigned
Identify documentation deficiencies and provide timely education and follow-up
Assist with ensuring physician/provider orders are appropriately implemented and followed
Monitor completion of required nursing assessments and documentation
Support accurate and timely communication of resident information across the interdisciplinary team
Assist with corrective action plans when documentation or clinical practice concerns are identified
Assist the Director of Nursing with maintaining ongoing survey readiness
Support nursing practices consistent with applicable federal, state, and facility requirements
Participate in facility rounds and identify potential clinical or regulatory concerns
Assist with preparation for regulatory surveys and other external reviews
Participate in survey processes and assist with responding to identified concerns
Support implementation and monitoring of plans of correction when applicable
Ensure nursing staff understand and consistently follow facility policies, procedures, and clinical expectations
Promote resident rights, dignity, privacy, safety, and freedom from abuse, neglect, and exploitation
Participate actively in the facility's Quality Assurance and Performance Improvement (QAPI) program
Review clinical trends and performance indicators with nursing and facility leadership
Assist with developing measurable improvement plans when opportunities are identified
Monitor the effectiveness and sustainability of corrective actions
Participate in facility quality, risk-management, and clinical meetings as assigned
Help translate identified quality concerns into education and operational improvements for frontline nursing staff
Identify nursing education and competency needs
Collaborate with nursing leadership and Staff Development to provide education and training
Provide hands-on coaching and mentorship to nursing staff
Assist with competency evaluations and clinical skills validation
Support new graduate nurses and employees transitioning into skilled nursing
Promote continuing education and professional development
Develop emerging nursing leaders and encourage advancement within the organization

Develop positive relationships with residents and families
Respond professionally and promptly to clinical questions and concerns
Participate in resident and family meetings when nursing leadership involvement is appropriate
Assist with resolving concerns before they escalate whenever possible
Communicate significant clinical concerns to appropriate leadership
Promote resident choice, dignity, independence, and person-centered care
Graduate of an accredited school of nursing
Current Registered Nurse (RN) license in the State of New Mexico in good standing
Current CPR/BLS certification preferred
Previous skilled nursing, long-term care, rehabilitation, or post-acute nursing experience strongly preferred
Previous nursing leadership, supervisory, Charge Nurse, Unit Manager, or ADON experience preferred
Strong clinical assessment and critical-thinking skills
Knowledge of skilled nursing clinical operations and regulatory expectations
Strong leadership, coaching, and employee-development abilities
Ability to address performance and accountability concerns professionally and consistently
Strong organizational and time-management skills
Ability to manage multiple priorities in a fast-paced clinical environment
Strong written and verbal communication skills
Experience with electronic health record systems
Ability and willingness to provide hands-on clinical leadership when necessary
