AT HOME CARE PHYSICIANS PLLC is looking for a compassionate and driven House Call Nurse Practitioner to join their team in Warrenville, IL. Responsibilities:

• Conduct comprehensive in-home patient assessments, evaluations, and follow-up visits
• Diagnose and manage acute and chronic conditions for homebound patients
• Develop and implement individualized care plans in collaboration with physicians and care teams
• Prescribe medications and order diagnostic tests as appropriate within scope of practice
• Coordinate with specialists, home health agencies, and other healthcare providers to ensure continuity of care
• Maintain accurate and timely clinical documentation in the EMR system
• Educate patients and families on disease management, medications, and preventive care

Requirements:

• Active Nurse Practitioner license in the state of Illinois
• Master's degree or higher in Nursing (MSN, DNP) with NP certification (FNP, GNP, or AGNP preferred)
• Experience in primary care, geriatrics, or home-based care strongly preferred
• Valid DEA license and ability to prescribe medications
• Reliable transportation and valid driver's license for travel to patient homes
• Strong clinical assessment and critical thinking skills in independent settings
• Excellent communication, empathy, and patient-centered bedside manner
