Inpatient RN Case Manager - Lead Care Coordination

Mayo Clinic in Phoenix seeks an experienced Registered Nurse Case Manager to coordinate patient plans across the continuum of care. Working with an interdisciplinary team, you will ensure appropriate utilization management, care coordination, and timely clinical documentation, within Mayo's nursing model.
