This role provides direct patient care and strategic leadership within a community and rural health setting, ensuring cohesive collaboration with the healthcare team and medical providers to deliver comprehensive, patient-centered services. As part of a Federally Qualified Health Center (FQHC), the Nurse Manager is responsible for upholding the highest standards of patient care, ensuring compliance with healthcare regulations, and addressing the needs of a diverse patient population. Functioning as a Clinic Nurse and departmental leader, the Nurse Manager supervises nursing staff, supports clinical workflows, and upholds care standards. They manage a team of nurses and exercise a high degree of independent judgment in overseeing daily nursing operations, often with minimal administrative oversight. This position reports directly to the Chief Medical Officer. Access, Stewardship, and Quality (ASQ) Access: The Nurse Manager ensures nursing and clinical services are accessible, equitable, and responsive to all patients. They streamline patient flow, coordinate care, and promote integration with medical, dental, and behavioral health services to provide seamless, whole-person care. Stewardship: The Nurse Manager fosters responsible and efficient use of resources while maintaining high-quality care delivery. Responsibilities include assisting with budget development, staffing oversight, and implementing cost-effective, evidence-based clinical practices. Quality: The Nurse Manager champions continuous quality improvement by monitoring clinical outcomes, maintaining regulatory compliance, and supporting professional development through mentorship, training, and performance evaluations. Additionally, the Nurse Manager provides safe, effective, culturally competent primary and preventive care, emphasizing health promotion and disease prevention. They supervise and support nursing staff across all Elevated Community Health locations and work closely with other healthcare professionals to ensure coordinated, comprehensive care. Supervise and mentor nursing staff, including RNs, LPNs, and medical assistants. Ensure adequate staffing levels and manage scheduling. Conduct performance evaluations and provide feedback to staff. Works directly with the Chief Medical Officer to develop all nursing-related programs. Maintain and develop nursing standard operating procedures (SOPs) and nursing policies and procedures. Provides education to clinical staff to promote staff and patient safety. Serves as nursing representative for CCHN. Serves as primary trainer and accountable representative for CCM (chronic care management) or oversees designee. Collaborate with the CMO and Medical Service Line Director to establish workflows incorporating nursing in team-based care. Foster TCM (transitional care management) workflow for better patient outcomes and decreased readmissions. Incorporate nursing-sensitive indicators (NSIs) piloted by AAANC (American Academy of Ambulatory Care Nursing) into RN workflows toward quality patient outcomes and team support for care coordination, PCMH, telehealth, chronic care, and population health. Develop and implement nursing competencies to benefit teams and increase patient access. Collaborate to establish and draft RN workflows to support team education. Education Design and implement education programs for clinic staff. Provide ongoing education and training to nursing staff. Optimize nursing as educators to improve patient self-management and encourage preventative care. Provide current, evidence-based care for clinical staff education and updates according to guiding body recommendations. Stay current with best practices and advancements in nursing care. Encourage RN professional growth opportunities for education, certification, and specialization. Promote a culture of continuous learning and professional development. Foster measures for RNs to promote preventative care through case management and education. Mentor RN team and serve as preceptor for nursing students to promote retention and recruitment. Clinical Oversee patient care plans and ensure they are executed effectively. Collaborate with physicians and other healthcare providers to optimize patient outcomes. Address patient concerns and complaints promptly and professionally. Oversee growth in the scope of practice for RN work in clinic settings. Support growth toward RN assessment and education. Optimize the nursing assessment to support providers. Ensure compliance with all federal, state, and local regulations. Education/Experience Must Haves A bachelor's degree in nursing (BSN) is required—a graduate of an accredited nursing school. Current Colorado RN license or current RN license from a participating compact state. Preferred knowledge of case management, immunizations, cultural diversity, and performance improvement. Exhibit cultural competence and exceptional teamwork skills. Ability to read, write, and speak the English language. Ability to interpret various instructions and deal with multiple linguistic and cultural variables. Knowledge of the industry's legal rules and guidelines. Nice to Haves Master's degree in Nursing (MSN) or Healthcare Administration (MHA) preferred. AAACN Certification in ambulatory nursing or equivalent preferred. At least 2 years of experience in ambulatory care. Previous non-profit and/or volunteer experience preferred. Ability to read, write, and speak Spanish (bilingual) is preferred.
