Licensed Vocational Nurse (LVN) – Home Health Shift: Day Shift – 5x8 hours (40 hours/week, Monday–Friday) Start Date: ASAP Contract Duration: 13 Weeks Bill Rate: $70/hr + Mileage Reimbursement ($0.725/mile IRS rate) Position Overview The LVN – Home Health provides skilled nursing care to patients in their homes, following physician orders and care plans. The role requires strong field‑based experience, productivity management, and efficient documentation using Home Health EMR systems. This is a high‑productivity, travel‑based role covering multiple communities and requiring flexibility to support additional territories if needed. Coverage Area Primary service areas include: Alief Stafford Sharpstown Meyerland May also cover additional areas within a ~60-mile radius from the facility Key Responsibilities Provide routine home health visits Deliver nursing care per physician orders and care plans Conduct patient assessments, basic skilled nursing procedures Maintain accurate and timely documentation Travel daily to patient homes across assigned territory Collaborate with care team and case managers Travel Requirements Daily field visits across assigned region Must be willing to travel up to 60‑mile radius Mileage reimbursed at $0.725/mile (IRS rate) Required Qualifications Experience Recent Home Health experience (2024–2025 required) Strong field experience in home health visits Certifications BLS/CPR (typically required for HH roles) Licensure Active LVN license (must be in hand) Mandatory: HomeCare HomeBase (HCHB) experience Candidates without HCHB may be considered at reduced pay for first 2 weeks if trainer is available.
