We are hiring experienced Registered Nurses for a specialized revisit-focused RN position throughout Alameda County and Contra Costa County designed around continuity of care, efficient workflows, strong support, and real work-life balance.

This role primarily focuses on skilled nursing revisits and ongoing patient management within a highly supported clinical model. Our nurses work alongside dedicated scheduling, intake, and clinical support teams so you are not left handling everything on your own after hours.

POSITION OVERVIEW

The Registered Nurse provides skilled nursing services in the home health setting including patient assessment, medication management, wound care, disease management, patient education, care coordination, and communication with physicians and interdisciplinary team members.

This role is ideal for experienced home health nurses looking for a more organized and well-supported field-based opportunity with efficient workflows and strong operational backing.

QUALIFICATIONS

Current California RN License required Home Health experience preferred Strong organizational and communication skills Ability to work independently in the field Reliable transportation and valid California Driver’s License
