CareRite Centers is seeking a dedicated Wound Care LPN to provide specialized treatment and monitoring for residents in our skilled nursing and rehabilitation centers. In this on-site role, you will perform comprehensive wound assessments, deliver evidence-based treatments, and collaborate with the interdisciplinary team to develop individualized care plans. You will document progress in the EHR, educate residents and families on wound care and prevention, and support a safe, dignified healing environment. This position offers the opportunity to grow your clinical skills in post-acute and long-term care while making a daily impact on resident outcomes.
