Description

• Conduct evaluations for patients in inpatient or outpatient settings and develop personalized treatment plans
• Implement physical therapy strategies targeting mobility, strength, balance, and overall function
• Work alongside nurses, physicians, case managers, and other rehabilitation professionals to maximize patient outcomes
• Guide patients and families in understanding therapeutic goals, discharge planning, and home exercise routines
• Document evaluations, treatments, and patient progress as per facility and regulatory requirements
• Engage in care team rounds and interdisciplinary meetings to ensure seamless continuity of care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications

• State licensure as a Physical Therapist
• CPR certification is a plus
• Specialized certifications (for example, in neurology or geriatrics) are beneficial, but not mandatory

Skills

• Prior experience is highly recommended
• Ability to work with patients facing complex medical and mobility challenges
