Seniors Helping Seniors® Provo Sandy is looking for a dependable, compassionate, and organized Care Coordinator to join our team.

This position plays an important role in supporting our clients, caregivers, and office team. The Care Coordinator helps ensure that client needs are understood, caregiver schedules are filled appropriately, and the highest level of care is provided.

Location: South Salt Lake County or North Utah County

The Care Coordinator will be responsible for:

• Working directly with caregivers to coordinate schedules, communication, and expectations

• Filling client shifts and helping ensure appropriate caregiver-client matches

• Understanding client needs and helping provide the highest level of care possible

• Conducting in-home visits for new client acquisition and care assessments

• Following up with clients regarding ongoing care needs and satisfaction

• Following up with caregivers to ensure quality of care and proper support

• Training on and using our CRM system, WellSky

• Using the company business phone app for all work-related communication

• Maintaining accurate reporting, documentation, and communication

• Supporting the overall mission and daily operations of the agency

The ideal candidate will be:

• Compassionate, professional, and dependable

• Organized and able to manage multiple priorities

• Comfortable communicating with seniors, families, and caregivers

• A strong problem-solver who can help fill shifts and respond to changing needs

• Comfortable learning and using technology, including CRM systems and phone apps

• Detail-oriented with strong follow-up and documentation skills

• Able to represent the agency professionally during in-home visits

Experience in home care, scheduling, caregiving, healthcare, customer service, or office coordination is helpful, but we are willing to train the right person.

This position starts at $20.00 per hour with an additional $1.00 per hour increases based on agency revenue growth.
