Job Summary

We are looking for individuals to join our client’s team as Community-based Licensed Master Social Worker responsible for providing direct mental health services, wellness support, treatment planning, and care coordination for patients enrolled in the ACT (Assertive Community Treatment) program.

Job Description

• Along with multi-disciplinary team, 68 clients engage in community and office visits at least once weekly to provide ongoing wellness and medication management, assessment, and other supports as needed
• Provides direct services related to the treatment and recovery of people living with mental illness according to ACT guidelines
• Demonstrate competencies in harm reduction, trauma-informed care, stage-wise treatment, motivational interviewing, and other evidence-based practices
• Displays core competencies in designated areas of practice, including the ACT core operating principles, integrated physical health and behavioral health, vocational services, family psychoeducation, and wellness self-management
• Coordinates treatment and services with a variety of other teams and systems including inpatient units, primary and specialty care physicians, supportive housing programs, substance use rehabilitation programs, respite care, etc.
• Completes goal-oriented treatment planning (plan of care, psychosocial, safety plans, etc.)
• Workload, utilization, number of patients, staffing structure, and hours of operation:
  • Social worker will manage caseload of 13-18 patients and will work on tour 2 (9am-5pm)
• Most patient care visits are conducted in the community (patients’ homes, a park, café, etc). Escorts are offered to benefits and medical appointments. We also offer significant primary care support.
• Documentation is typically completed onsite in Program offices.

Skills Required

• Minimum of one (1) year of experience as a Licensed Master Social Worker (LMSW).
• Experience in community mental health, Assertive Community Treatment (ACT), harm reduction, trauma-informed care, motivational interviewing, behavioral health, care coordination, and treatment planning.
• Ability to conduct weekly community visits and manage a caseload of 13–18 patients.
• Skilled in providing medication and wellness support, developing treatment plans, coordinating with multidisciplinary teams, and maintaining accurate documentation.
• Strong crisis intervention and behavioral health support skills.
• Excellent communication, organizational, and documentation abilities.
• Compassionate, patient-focused, and comfortable working independently in community-based settings.
• Collaborative team player with the ability to build positive relationships with patients, families, and healthcare professionals.

Education/Training/Certifications

• NYSED LMSW License
• Master’s degree in social work
• Primary Source Verification
• SEL/SCR Certification

Pay Rate

• $51–$60 per hour, based on experience, skills, and market factors
