What You'll Do
- Provide outpatient physical therapy services to home health patients
- Conduct assessments and develop personalized care plans
- Implement therapeutic interventions for functional improvement
- Educate patients and family members on exercises, mobility, and safety
- Collaborate with medical teams to ensure optimal patient outcomes
- Document patient progress using Meditech Home Care EMR system
- Travel within a 50-mile radius to serve various patient locations
What You Need
Required:- Valid Physical Therapist license for Louisiana
- BLS certification
- At least 1 year of recent acute care experience
- Valid driver's license and reliable transportation
- Experience with wound care, heart failure, diabetes, IV antibiotics, Foley, and PICC line management
- Experience in medical/surgical settings, ER, or ICU
