Outpatient Physical Therapist

This is a 13-week contract position.

5x8.0 shifts, flexible hours, with 1-2 evenings until 7 PM.

The Outpatient Physical Therapist is responsible for evaluating assigned patients and developing, implementing, and modifying treatment plans based on individual needs, goals, and physician direction. This role includes the delegation and direction of all discipline-specific therapy services for assigned patients, ensuring the design of functional treatment programs for effective community reintegration.

Responsibilities:

• Complete comprehensive evaluations of patients to develop appropriate treatment plans and goals.
• Document all care in a clear, concise, accurate, and timely manner, ensuring compliance with state and federal guidelines.
• Provide care based on signed physician orders and insurance approvals.
• Participate in student training programs as assigned, fostering a positive learning environment.
• Engage in professional development activities, including continuing education and self-study.
• Adhere to state practice laws and maintain professional licensure in good standing.
• Contribute to quality improvement initiatives and actively participate in performance improvement activities.

Required Experience / Certifications / Licensure:

• Degree in Physical Therapy from an accredited institution.
• 1+ years of outpatient clinical experience.
• Current CPR certification.
• Active and applicable state licensure for physical therapy.
