Hospice Hospital Liaison RN — Care, Referrals & Growth

Constellation Hospice is seeking an energetic Registered Nurse (RN) to manage General Inpatient hospice patients in hospitals and to perform starts of care and revisits. The Hospital Liaison will expand business opportunities by building relationships with social workers, physicians, and hospital administration. This on-site role requires strong assessment, documentation, and communication skills, with a focus on consent, ordering appropriate meds and DME, and ensuring admissions and transfers #J-18808-Ljbffr Constellation Health Services
