We are dedicated to providing the best resident centered care.Job Summary: We are seeking a skilled and motivated Occupational Therapist (OT) to join our dynamic rehab and home health team. In this role, you will assess, plan, and implement individualized therapy programs for both short-term rehab patients and home health clients. The ideal candidate is a caring professional who enjoys working in a team environment and is committed to helping individuals regain or maintain their independence in daily living.Key Responsibilities:Evaluate patients' functional abilities and develop individualized treatment plansProvide hands-on therapy to improve residents' and patients' ability to perform daily activities (ADLs)Document progress and adjust treatment plans as neededEducate patients, residents, and caregivers on exercises and strategies to improve functionCollaborate with interdisciplinary team members, including physical therapists, speech therapists, nursing staff, and social workersEnsure compliance with regulatory requirements and documentation standardsMaintain accurate records using electronic medical record (EMR) systemsQualifications:Bachelor's or Master's degree in Occupational Therapy from an accredited programCurrent state licensure as an Occupational Therapist (OTR/L)Excellent communication, organization, and time management skillsCompassionate, patient-centered mindset with a passion for helping others
