Develop, implement, and monitor individualized treatment plans aimed at improving mobility, strength, balance, gait, and functional independence. Provide direct patient care through therapeutic exercises, modalities, and restorative interventions, while ensuring safety and infection control protocols. Document patient progress, update treatment plans, and liaise with the facility’s interdisciplinary team to support discharge planning and care transitions. Educate residents and families on home programs, fall prevention, posture, and activity modification; ensure clear and accurate clinical notes.
