The Care Team Hospice is seeking a Registered Nurse Case Manager to provide skilled nursing care and coordinated end-of-life support to patients in their homes and facility settings while collaborating closely with the interdisciplinary team.Schedule, Hours & Territory• Standard operating hours are Monday through Friday, 8:00 AM to 4:30 PM• Wayne County, MI• RN Case Managers may participate in an on-call rotation with additional pay• Some branches offer dedicated on-call staff, resulting in backup on-call coverage onlyWhat We Offer• Strong interdisciplinary team collaboration• Supportive clinical leadership and resources• Clear expectations and consistent workflows• Environment focused on quality, compliance, and patient-centered careBenefits• Competitive Total Care Benefit Package, compensation, and mileage reimbursement per company policy• 401(k) with employer match• Health Savings Account eligibility with employer contribution up to $1,000 annually• Employee Assistance Program, discounts on gym memberships and wellness programs• Tuition reimbursement and Certification Assistance• PTO begins accruing upon hire, with full-time team members earning up to 3 weeks of PTO during their first year of employment and increasing accruals with tenure, plus seven paid holidays.Key Responsibilities• Conduct comprehensive patient assessments and develop individualized plans of care• Provide skilled nursing care in accordance with hospice standards and physician orders• Coordinate care with physicians, caregivers, and interdisciplinary team members• Educate patients and families on symptom management, disease progression, and end-of-life care• Document assessments, interventions, and care plans accurately and in a timely manner in the EMR HCHB (Homecare Homebase)• Participate in interdisciplinary team meetings and care planning• Support after-hours patient needs as part of an on-call rotation when applicable• Active Registered Nurse (RN) license• Hospice or home health experience preferred• Strong clinical assessment and care coordination skills• Ability to work independently in the field• Strong communication and documentation skills• Experience using HCHB (Homecare Homebase) preferredField Role Requirements• Active driver’s license• Ability to pass a criminal background check• Ability to pass a Motor Vehicle Record (MVR) check• Current TB testing in accordance with agency policy• Current CPR certification as required for the rolePay: $80,000.00 - $92,000.00 per yearBenefits:* 401(k) matching* Employee assistance program* Health insurance* Health savings account* Life insurance* Paid time off* Referral programWork Location: In person #J-18808-Ljbffr
