Our coordinated 360 approach also gives families peace of mind knowing their loved ones are receiving dependable and compassionate care.Position SummaryThe Registered Nurse will assess client needs, develop and update individualized Plans of Care, provide skilled nursing services, and help ensure that care meets professional, regulatory, and agency standards. The RN may also supervise and support Certified Home Health Aides and collaborate with physicians, therapists, caregivers, and other healthcare professionals to provide coordinated care.ResponsibilitiesConduct initial and ongoing in-home nursing assessmentsDevelop, review, and update individualized Plans of CareProvide direct nursing care based on client needsMonitor clients and identify changes in conditionCommunicate significant findings to families, physicians, and the care teamComplete accurate and timely clinical documentationCoordinate services with healthcare professionals and caregiversConduct supervisory visits and evaluate care provided by home health aidesProvide education and clinical guidance to caregivers and familiesFollow infection-control, safety, HIPAA, and agency requirementsReport incidents, concerns, and changes in client conditionQualificationsActive New Jersey Registered Nurse license - In good standingCurrent CPR certificationStrong clinical assessment and critical-thinking skillsHome care, home health, geriatric, rehabilitation, or community nursing experience preferredExperience supervising Certified Home Health Aides preferredKnowledge of medication management, infection control, documentation, and care planningCurrent negative TB test or chest X-rayAbility to complete all required background checks and screeningsExcellent communication, organization, and customer-service skillsCompassionate, dependable, professional, and detail-orientedAbility to work independentlyComfortable using technology for scheduling and clinical documentationAbility to travel throughout Bergen CountyValid driver's license and reliable transportation preferredMeaningful Care Starts with YouAt Qualicare, nursing is more than providing clinical services.
