Responsibilities may include Provide clinical oversight Intake assessments, conduct behavioral health intakes and psychosocial assessments Team resource support within a regional BHI team serving skilled nursing and long-term care (SNF/LTC) settings. Support Value-Based Care initiatives, and serve as a clinical Ideal for clinicians who enjoy systems-level impact, Interdisciplinary collaboration, and shaping a growing behavioral health program. Requirements for this position include: At least 18 years of age must be licensed as an LPCC, LPCC-S, LISW, LISW-S or IMFT Master’s degree in social work, professional counseling or marriage and family therapy; license must be maintained during employment. Clinical experience treating individuals with a variety of diagnoses and presenting issues is preferred. Must be Medicare-eligible to provide intake and assessment services and possess a minimum of three (3) years of behavioral health, case management, or related clinical experience. Experience working in skilled nursing facilities, long-term care settings, geriatric populations, Value-Based Care programs, and/or Behavioral Health Integration (BHI) programs is preferred. Experience conducting diagnostic assessments and behavioral health intake preferred. Prior team lead, supervisory, or clinical leadership experience is also preferred. Must be responsible, mature and flexible; must possess excellent decision-making, time management and communication skills Must be able to respond calmly to crisis situations; demonstrate the ability to direct others in a confident, respectful manner and exercise good judgment in interactions with residents, families, employees, peers and regulatory agency personnel. Must appropriately handle confidential information. Must have a valid driver's license and reliable transportation. Bilingual language skills are a plus.
