Responsibilities
- Perform physical therapy evaluations and treatments for a mix of inpatient and outpatient cases.
- Support mobility progression from acute care through SNF, including discharge planning and coordination with care teams.
- Provide therapy in ER, ICU, Med/Surg (Acute, Observation, and SNF), with occasional work in OB and extensive activity in the hospital-based outpatient clinic.
- Maintain a full schedule with 30–45 minute outpatient visits and 1-hour new evaluations; typically 5–15 patients per day.
- Document treatment plans, progress notes, and discharge summaries; communicate with physicians, nurses, and case managers to optimize patient outcomes.
- Demonstrate independence in a rural Kauai setting, adapting approaches to diverse outpatient diagnoses and ensuring continuity of care.

Requirements
- Hawaii Physical Therapy license required (ability to obtain promptly)
- Ability to work across inpatient and outpatient settings (ER, ICU, Med/Surg, SNF) and hospital-based outpatient clinic
- Availability to work Saturdays as required by the assignment
- Experience with inpatient mobility progression and discharge planning is preferred
- Strong clinical judgment, autonomy, and commitment to patient-centered care
