divh2The Jefferson - Health Care Center/h2pThe Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN) is responsible for providing direct resident care and supervision of direct resident care of non-licensed team members in the skilled nursing operations and other areas throughout the community as appropriate. with the development and leadership of the Designated Nursing Assistants (DNA)/Certified Nursing Assistants (CNA) and follows the Resident Centered Care model./liliAssist with providing clinical care and not limited to urinary catheterization, tube feedings, applying/changing dressings, Accu-checks, medication management/administration, admission, and transfer and discharge of guests/residents./liliComplete timely and accurate documentation of guests/residents charts./liliComplete Incident Reports and report to Director of Nursing Services (DNS)./liliReview, update, and notate in the electronic health record to learn and document pertinent resident and community information./liliConduct effective visits/rounds at a minimum twice a day and conduct visits/rounds with physicians as necessary as assigned by DNS./liliResearch and document all incidents of abuse, neglect, and misappropriation of funds as assigned by DNS, Executive Director (ED), or Skilled Nursing Administrator (SNA)./liliParticipate in the Clinical IDT meeting and in the Daily Prospective Payment System (PPS) meetings to obtain information on admissions and discharges for the day/week./liliAssist in monitoring medication records and administration./liliAssist in receiving lab, x-ray, and other test results and communicates appropriately./li/ulpQuality Assurance and Regulatory Compliance/pulliStrive for excellent quality care and service delivery as measured in the Quality Services Review (QSR) process in demonstration and knowledge of standard practices and excellent resident care and services./liliDemonstrate and is knowledgeable in the following key quality improvement areas:/liulliSkilled nursing transformation and Resident Centered Care Model./liliSunrise and Federal Quality Indicators./liliActively participates in Sunrise Quality Indicator Meetings./liliAssists with development of plans for Quality Indicators above the Sunrise Assessment Instrument process./liliActively participates in Federal, state/provincial, and Local surveys./li/ul/ulpRestorative/pulliFollow the documentation guidelines of the Restorative process./liliTrain and monitor the DNAs/CNAs in the Restorative program./liliMonitor the documentation for the Restorative program./liliMonitor for resident decline and improvement./liliRefer guest/resident to restorative nursing as indicated./li/ulpClinical Systems/pulliDemonstrate Sunrise practices and guidelines for care and services surrounding guest/residents skin, nutrition, behavior, falls, and bowel/bladder./liliAssist in implementation, training, and monitoring of documentation related to clinical systems weekly./liliConsult with physician as appropriate./liliEnsure guest/resident changes, concerns, and/or solutions are reported to the DNS as appropriate./li/ulpMedication Management/pulliTranscribe physician orders in the Medication Administration Record/Treatment Administration Record (MAR/TAR) accurately and promptly to include initials, date and time noted, and date faxed to pharmacy./liliDemonstrate and is knowledgeable of the Six (6) Rights of Medication Pass (Right resident, medication, dosage, time, route, right to refuse)./liliPerform and follow Sunrise and nursing standards for taking orders from physicians and administering medications./liliEnsure current guest/resident photo with dates for all guest/resident on MAR/TAR./liliComplete Med Pass audit as a quarterly minimum./liliComplete MAR/TAR audit each month./liliMonitor the Medication Room, medication carts, and treatment carts for neatness, cleanliness, availability of medications, and expired medications./li/ulpGuest/ Resident, Family, and Visitor Relations/pulliRespond to guest/resident, family, and visitor requests in a timely and professional warm manner./liliCommunicate with families regarding the guest/resident medical care and seek new methods to incorporate guest/resident requests into existing nursing standards./liliMaintain confidentiality of all guest/resident information./liliReview, read, notate, and initial the Daily Log to document and learn about pertinent information and any residents physical and behavioral changes./li/ulpRisk Management and General Safety/pulliPartner with community team to ensure community follows national/provincial regulations pertaining to occupational health and safety requirements, promoting Risk Management programs and policies and adhering to safety rules and regulations./liliPractice safety procedures at all times including Personal Protective Equipment (PPE), fire extinguishers, Safety Data Sheets (SDS), and Lockout Tagout procedures./li/ulpInfection Control/pulliUnderstand and follow infection control practices./liliProvide training as needed to support infection control practices./liliUnderstand and practice the proper method of attending to and disposing of, and the possibility of exposure to, blood borne pathogens, bodily fluids, infectious waste, sharp sticks, and hazardous materials./liliReport occupational exposures to blood, body fluids, infectious materials, sharp sticks, and hazardous chemicals immediately./liliEnsure oxygen tanks are stored safely, exchanges guest/resident tanks when empty, and monitors to make sure liters of oxygen are at prescribed levels./li/ulpSupervision/pulliAssist with staffing and scheduling ensuring staffing is sufficient to meet guest/resident needs./liliAssist with providing team member evaluation feedback, coaching, and counseling on care provided as appropriate./liliProvide leadership for team members during orientation, Sunrise University trainings, and evaluations as
