Full Time Professionals Laurel, MS, US 3 days ago Requisition ID: 1974 Medical Social Worker Job Title: Medical Social Worker Department: SCH Social Services Full Time/PRN: Full Time Job Summary Provides psychosocial assessment, counseling, advocacy, and coordination of community resources for patients and families. Supports continuity of care, discharge planning, and emotional well-being in the hospital setting. Minimum Qualifications Bachelor’s degree in Social Work required Licensed or license-eligible per state requirements Basic Life Support (BLS) certification Knowledge of community resources and discharge planning Preferred Qualifications Hospital or healthcare social work experience preferred Conduct comprehensive psychosocial assessments for patients and families Provide counseling, crisis intervention, and emotional support Coordinate discharge planning and post-acute community resources Collaborate with physicians, nurses, and interdisciplinary care teams Advocate for patient rights and continuity of care Maintain accurate, timely clinical documentation Physical Requirements Ability to stand and/or walk for extended periods. Frequent bending, reaching, squatting, and kneeling. Lift, carry, push, and pull up to 50 lbs independently; >50 lbs with assistance/devices. Assist with patient repositioning and transfers using proper body mechanics and equipment. Manual dexterity for clinical procedures and computer use; adequate vision, hearing, and speech.
