Southern Tier Connect is seeking a Care Coordination Manager in Cortland, NY to oversee individualized Life Plans for people with I/DD. You will coordinate interdisciplinary teams, schedule meetings, and ensure access to Medicaid and support services, while maintaining person-centered planning and consumer rights.
