Key Responsibilities\n- \n
- Provide clinical assessment, diagnosis, and treatment planning for clients experiencing major mental illnesses or severe emotional disorders. \n
- Develop and maintain a manageable caseload, facilitating individual, group, and family therapy sessions as needed. \n
- Create and implement integrated care plans, ensuring timely access to resources and appropriate support services for clients and their families. \n
- Conduct brief, evidence-based screenings to determine the need for more comprehensive evaluations. \n
- Support clients through crisis intervention, addressing immediate needs and long-term mental health concerns. \n
- Collaborate actively with team members to meet and exceed shared objectives, participating in ongoing team-related discussions and meetings. \n
- Ensure adherence to ethical guidelines and meet documentation standards for reporting and data submission. \n
- Monitor and evaluate treatment progress, making adjustments to therapy approaches as required to optimize client outcomes. \n
- Participate in committees focused on improving service delivery and enhancing the overall quality of care. \n
- Maintain an awareness of safety protocols and identify any unsafe situations to safeguard clients and colleagues. \n
This role primarily involves working with patients enrolled in hospice care, specifically those dealing with terminal illnesses and complex emotional challenges. - A Master’s degree in Social Work from an accredited program and current LCSW licensure in Illinois. \n
