What You Will Do

• Practice independently in the community as part of an interdisciplinary care team.

• Deliver personalized nursing and care management to patients in their home or care facilities.

• Constantly evaluate evolving patient needs and respond with plan of care adjustments.  

Licenses and Certifications:
A license and current registration to practice as a Registered Professional Nurse in New York State required and Education:
Bachelor's Degree in Nursing OR Associate’s degree in Nursing required Work Experience:
Four years’ clinical home care experience required and
Minimum of two years full-time clinical home care experience required and
Minimum of one year of experience as a field staff nurse in VNS Health Home Care, which includes ability to manage/document cases with a minimum of supervision, identify/assess/formulate/implement action plan to address problem and evaluate outcome preferred and
Pay Range USD $126,413.00 - USD $160,188.00 /Yr.
