Job Title: Licensed Practical Nurse (LPN) Care Coordinator

Job Description

This role offers an excellent opportunity for a Licensed Practical Nurse to provide comprehensive care coordination and patient support in a family medicine clinic setting in Norfolk. You will play a key role in managing patient visits, ensuring accurate documentation, and facilitating seamless communication between patients, providers, and the broader healthcare team. The position focuses on patient intake, medication management, prior authorizations, and ongoing care coordination, while supporting a collaborative, growth-oriented clinical environment.

Responsibilities

• Manage patient visits from intake through completion, including assessing chief complaints and gathering relevant clinical information.
• Perform patient intake activities such as verifying personal and contact information, assessing smoking status, and reconciling medications and allergies.
• Prepare exam rooms prior to patient visits, ensuring rooms are clean, organized, and stocked with necessary supplies.
• Restock clinical areas regularly and ensure medical equipment is properly sterilized, maintained, and ready for use.
• Document all nursing interventions, assessments, and patient interactions accurately in the electronic medical record.
• Review patient charts for completeness and accuracy, and obtain necessary medical records from internal and external sources as needed.
• Handle prior authorizations for medications, procedures, and services in coordination with providers and insurance requirements.
• Manage medication review and refill requests, ensuring appropriate clinical and documentation standards are met.
• Respond promptly and professionally to patient messages, including questions about care plans, prescriptions, and test results.
• Process and communicate lab results, ensuring timely follow-up and coordination with providers and patients.
• Coordinate referrals to specialists and other services, tracking status and ensuring continuity of care.
• Support care management and case management activities, helping to coordinate services and resources for patients with ongoing healthcare needs.
• Conduct patient satisfaction surveys and follow-up assessments to support quality improvement and patient experience initiatives.
• Adhere strictly to infection control protocols, environmental safety standards, and patient confidentiality requirements.
• Complete annual recertification and training requirements, including OSHA, compliance, respiratory fit testing, and IT policy training.
• Collaborate with providers, nurses, and other clinical staff to ensure high-quality, patient-centered care in a family medicine environment.
• Use electronic health record systems effectively for chart review, documentation, triage, and care coordination tasks.
• Assist with triage activities as appropriate, helping prioritize patient needs and directing them to the appropriate level of care.
• Contribute to a supportive learning environment by engaging in continuous professional development and sharing best practices with the team.

Essential Skills

• Active Practical Nurse License (Licensed Practical Nurse, LPN).
• Current Basic Life Support (BLS) certification.
• Demonstrated experience in nursing and direct patient care in a clinical setting.
• Proficiency in care management and care coordination within a healthcare environment.
• Experience working with medical records, including chart review and documentation.
• Ability to use electronic health record (EHR) systems efficiently for documentation and care coordination.
• Knowledge of infection control, environmental safety, and patient confidentiality standards.
• Skills in triage, case management, and managing referrals and prior authorizations.
• Strong organizational skills for managing multiple patient visits, documentation tasks, and follow-up activities.
• Effective communication skills for interacting with patients, providers, and other members of the healthcare team.
• Attention to detail when reconciling medications, allergies, and clinical information.
• Ability to handle prescription refills and lab results in accordance with clinical protocols.

Work Environment

This position is based in a family medicine office in Norfolk within a large healthcare organization. The clinic environment offers exposure to a wide range of conditions and treatments, providing rich learning opportunities and the potential to grow into permanent roles and advance professionally across various care settings. The team culture emphasizes learning, skill development, and collaboration between managers, providers, and clinical staff. You will work primarily with electronic health record systems and standard clinical equipment in exam rooms and patient care areas. The schedule does not include holidays or weekends, supporting work-life balance. Paid time off and sick time are available, and the setting encourages a professional clinical appearance appropriate for a family medicine practice.

This is a Contract to Hire position based out of Norfolk, VA.

The pay range for this position is $28.00 - $32.00/hr.

Individual compensation offered for this position within this range will depend on many factors, including qualifications, skills, relevant experience, job knowledge, geographic location, internal equity, and other pertinent job-related factors.
