OUTCOMES

The Pathway of Hope Case Manager is responsible for managing Pathway of Hope casework services to families at the Lawn Corps. The Case Manager works under the guidance of the Regional Social Work Manager along with the Corps Officer(s) implementing the program's objectives, that is, providing services to Pathway of Hope families, keeping required records, and identifying resources to support participating families.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Provides casework services to ensure the full implementation of the Pathway of Hope model within the assigned Corps. (the POH Case Manager in this program can only do Pathway of Hope responsibilities).

Program Implementation

  1. Provides intake and assessment services as defined by the Pathway of Hope curriculum.
  2. Deliver the full range of Pathway of Hope individualized services for a minimum of 20 families annually including:
    1. Conduct pre-screening and intake of participants using specified assessment tools. Families must meet low-income guidelines with an emphasis on vulnerable populations within the surrounding communities.
    2. Provide linkage to Pastoral Care Representative for pastoral care, counseling, and support.
    3. Conduct goal setting steps with participants formulating change-oriented action plans.
    4. Conduct follow-up case management meetings with participants.
    5. Refer participant to internal and/or external services designed to address identified goals.
    6. Monitor and track participants progress in goal attainment on the action plans.
    7. Develop plans to address transition needs and ongoing documentation of progress.



  3. Enters client data and reporting of information into the SIMS system that supports local, national, and territorial POH program outcomes.
  4. Maintains case files for each client that includes all assessments, action plans, case notes, graduation plans.
  5. Participates with POH planning and program development at assigned sites.
  6. Participate in scheduled cluster meetings, conference calls and individual consultation/supervision meetings with the Regional Social Work Manager.
  7. Participates in meetings with Corps Officer(s) and other Corps staff.
  8. Participates in Divisional or Territorial educational forums and webinars designed to facilitate understanding of a wide range of topics (especially related to case management and POH).
  9. Maintains working relationships with community agencies to provide comprehensive services for participants and to stay abreast of current trends and resources available.
  10. Conduct outreach initiatives during pantry distribution times, within the community, at community meetings, etc.
  11. Develop and participate in outreach and community partnership initiatives to strengthen community engagement and access to services, including participation in community-based activities and meetings.


Pathway of Hope Program Evaluation and Outcomes Measurements

  1. Ensure the accuracy of data entry into the database.
  2. Supports the POH implementation plan as requested and/or assigned.
  3. Serves on committees related to case management, emergency assistance and building sufficiency, as requested or appointed.


Program Evaluation and Certification

  1. Keeps records in compliance with standards in order to assure for program evaluation/certification requirements.

REPORTING RELATIONSHIPS

This position reports to the Corps Officer(s) with guidance from the Regional Social Work Manager.

This position will work in conjunction with the Program Services Analyst and Regional Social Work Manager to support reporting and data collection needs, when necessary.

This position works collaboratively with Corps Officer(s), and social services with the goal of ensuring effective delivery of quality services within the context of mission objectives.

In contacts related to this position's duties, this individual acts as a representative of The Salvation Army and its mission.

PERFORMANCE MEASUREMENTS

This individual will be evaluated on how effectively the outcomes of this position are achieved, as well as the timeliness, accuracy, and completeness of accomplishing assigned goals.

A 30, 60, and 90-day performance review will be conducted.

EDUCATION/EXPERIENCE

  • Bachelor's degree in a human service area required. A BSW is preferred.
  • Minimum of two years of experience in casework or social service.
  • A valid driver's license and clearance of The Salvation Army's MVR process.
  • Bilingual (English/Spanish) with an ability to communicate fluently in Spanish required.
  • Complete the Territorial Caseworker Certification Program, within the first 90 days of hire.

POSITION LIMITATIONS

  • This individual will only commit Army resources that have been allocated or approved.
  • This individual will keep the Regional Social Work Manager, and the Corps Officer(s) informed on all critical issues relating to their area of responsibility.
  • This individual will adhere to all policies and procedures in carrying out the responsibilities of this position.


PHYSICAL DEMANDS/WORK ENVIRONMENT

  • This position is required to do moderate physical work and must be able to lift 25 lbs.
  • In order to successfully perform the essential functions of this position, the employee is regularly required to use standard office equipment and PC.
  • The work environment for this position includes an office environment with a low noise level.
  • Must be able to travel to assigned service sites.
  • Maintain a positive work atmosphere by behaving and communicating in a manner that fosters and maintains positive relationships with co-workers and supervisors. This would include but not be limited to such actions: resolution of conflicts in a positive manner; courteous treatment of staff, clients, and visitors; respect for others' property and person, professional and appropriate communication to and about co-workers, supervisors, and subordinates.
