Social Worker PRN

Job Overview Saint Luke's North Hospital is seeking a PRN Social Worker to join our team! The role involves assessing psychological, social, emotional, cultural, or financial needs that impact patient health and recovery, providing professional social work services utilizing supportive counseling and clinical knowledge to assess patient and family needs during illness, crisis intervention, post‑discharge planning, collaborating with the healthcare team on transition plans, and addressing needs promptly. Responsibilities Assess psychological, social, emotional, cultural, or financial needs affecting patient health and recovery. Provide professional social work services with supportive counseling and clinical knowledge during illness, crisis intervention, and post‑discharge planning. Collaborate with the healthcare team to develop transition plans and address needs in a timely manner. Participate in program planning and development to meet the needs of patients and the community. Participate in daily treatment team meetings. Complete family therapy sessions with patients and families. Provide psychoeducational group therapy sessions. Assist with discharge planning, referrals, and transportation arrangements. Qualifications Master’s in Social Work (MSW) required. LMSW or LCSW in Missouri required. Licensed Master’s Social Worker in MO (less than 1 year experience). Work Schedule PRN; shifts available any day of the week, hours 8 a.m. to 4:30 p.m. Company Saint Luke’s Health System is the only faith‑based, nonprofit, locally owned health system in Kansas City, with 18 hospitals and campuses across the region.
